2.  Fill out an application http://www.odjfs.state.oh.us/forms/num/JFS%2007200/ and return through:

       a. Fax – Fax your signed application to 330-287-5899

       b. Mail – Signed applications can also be mailed to the address below.

       c. Deliver in person- Signed applications can be dropped off between the hours of 7:30 AM and 4:30 PM at:

Authority responsible for assistance to low-income families with children as well as Medicare services, food stamps, and daycare. Responsibilities include assisting recipients in receiving training and finding employment.
